Cholangioscopy (SpyGlass)
Cholangioscopy (SpyGlass) in Istanbul: when a closer look helps
Unimed Editorial Team · · 4 min read
Understand cholangioscopy for uncertain bile duct narrowing or difficult stones, with risks, alternatives and follow-up.

The short version
• Cholangioscopy provides a direct view inside the bile duct, usually during ERCP.
• It may help with targeted tissue sampling or breaking up difficult stones.
• It does not guarantee a diagnosis, replace every other test or cure cancer.
What is SpyGlass cholangioscopy?
During ERCP, a flexible endoscope passes through the mouth into the upper small bowel. A much thinner scope can then enter the bile duct through that endoscope. This is cholangioscopy; SpyGlass is a named system used for this approach.
The extra view can guide biopsy tools or instruments used to fragment difficult stones. It is not simply a scan, and it requires a specialist procedure team. St George’s patient information

Who might benefit from a closer look?
One situation is an indeterminate stricture: a narrowed bile duct whose cause remains uncertain. ESGE suggests cholangioscopy-guided biopsies alongside standard ERCP diagnostic methods in this setting. The location of the narrowing, previous samples and imaging all affect the investigation plan. A stricture is not automatically cancer. ESGE diagnostic guideline
Another situation is a stone that is too large or difficult to remove using usual ERCP techniques. Under direct vision, the team may break it into smaller pieces before removal. This is different from assessing a suspicious narrowing. University Hospitals Sussex
Ask: “Is the goal to clarify a diagnosis, clear a stone, improve drainage—or more than one of these?”
Could another test be more appropriate?
MRI/MRCP, CT or ultrasound may provide information without entering the duct, although they cannot perform the same treatment. Some patients need a different drainage approach if endoscopic access is unsuccessful. St George’s patient information
EUS and ERCP-based sampling can be complementary rather than competing options. ESGE recommends combined EUS-guided and ERCP-based tissue acquisition for selected patients with jaundice and a distal extrahepatic stricture without a pancreatic mass. That recommendation does not apply automatically to every narrowing. ESGE diagnostic guideline
The useful question is not “Which test is newest?” but “Which test would change the next decision?”
Does it diagnose or treat cancer?
Cholangioscopy can help obtain tissue from an abnormal area. Seeing a suspicious surface is not the same as receiving a pathology diagnosis.
A negative or inconclusive sample may still require further assessment. ASGE particularly discusses cholangioscopic or EUS-guided sampling after nondiagnostic investigations, with the choice influenced by the stricture’s location and other findings. A cancer treatment plan requires a separate specialist assessment; the camera itself is not a universal cancer treatment. ASGE guideline summary

What risks should we discuss?
Possible complications include pancreatitis, bleeding, a tear in the digestive tract, bile duct infection and problems related to sedation. Some complications require admission or further intervention. Personal risk depends on the planned steps and your health. St George’s patient information
Infection prevention and adequate bile drainage matter. ESGE suggests antibiotic prophylaxis when performing cholangioscopy and describes measures to reduce post-ERCP pancreatitis. The treating team decides what is suitable, including contraindications; do not self-prescribe antibiotics or change medicines from this article. ESGE adverse-event guideline
What happens before and after the appointment?
Request written fasting and medication instructions. Tell the clinical team about blood thinners, diabetes medicines, allergies, previous procedures and any existing stent. Do not stop prescribed medicines without their advice.
Before leaving, clarify who will explain any biopsy result and when. If a temporary stent is placed, obtain a documented removal or exchange plan. Arrange an escort and follow the unit’s sedation restrictions. St George’s patient information
Seek urgent medical assessment for severe or worsening pain, fever with jaundice, persistent vomiting or black stools. Do not wait for a travel coordinator to answer. University Hospitals Sussex
How should I plan an assessment in Istanbul?
Before booking travel, ask which records are needed and how to share them securely. Useful questions include:
• Has the specialist reviewed the actual imaging and earlier procedure reports?
• Is the proposed visit an assessment or a confirmed procedure?
• Who will handle urgent problems, pathology follow-up and any repeat treatment?
• Can follow-up and stent care be coordinated with my clinician at home?
Discuss accommodation and return travel with the treating team rather than relying on a fixed package timetable. Hospital, equipment, clinician availability and suitability must be confirmed individually.

Frequently asked questions
Is it the same as ERCP?
Usually it is an additional direct-view technique used during ERCP, not a substitute for the entire procedure.
Will everything be completed in one visit?
That cannot be promised. Tissue results, drainage needs or difficult stones can require further appointments.
Can I arrange it before a specialist reviews my case?
Start with assessment. A named technology alone does not establish that you need it.

Have reports for this procedure?
Send them for a confidential specialist review. A coordinator confirms what is missing and when the doctor's view will be ready.
