Single- and double-balloon enteroscopy
Single and double balloon enteroscopy in Istanbul
Unimed Editorial Team · · 4 min read
Questions about balloon-assisted enteroscopy, alternatives, preparation, risks and small-bowel assessment in Istanbul.

The short version
• Capsule endoscopy takes pictures; balloon enteroscopy can also allow tissue sampling or selected treatment.
• The planned target and your previous tests help determine the route.
• A complete examination or successful treatment is not guaranteed.
What are single and double balloon enteroscopy?
Both are forms of device-assisted enteroscopy, using balloon support to help advance a flexible endoscope through the small bowel. The equipment differs, but the patient’s key question is the same: what area needs to be reached, and what will the specialist do there?
The scope may enter through the mouth or through the anus. Previous investigations guide this choice. Sedation or general anaesthesia and day-case versus inpatient care depend on the procedure, your health and the unit’s arrangements. ESGE technical review, 2018

When might it be useful?
ESGE recommends device-assisted enteroscopy to confirm and possibly treat lesions found on small-bowel capsule endoscopy. For suspected small-bowel bleeding, capsule endoscopy is generally the first-line examination. Enteroscopy is therefore not automatically the first test for unexplained symptoms or anaemia. ESGE guideline update, 2022
Depending on the findings and consent, the clinician may take biopsies, treat a bleeding area or remove selected polyps. Not every abnormality is suitable for treatment during the same session. King’s College Hospital patient information
A useful question is: “What finding are we trying to confirm, and how would the result change my care?”
How does it differ from a capsule or scan?
A swallowed capsule provides images without the same opportunity to take tissue or perform treatment. CT or MRI may answer different questions. The best sequence depends on what is already known.
For suspected Crohn’s disease, ESGE places conditions on capsule use, including the absence of obstructive symptoms or known narrowing in the relevant diagnostic pathway. In established Crohn’s disease, it recommends a patency capsule before small-bowel capsule endoscopy to reduce retention risk. Do not arrange a capsule independently if narrowing is a concern. ESGE guideline update, 2022
Ask whether reviewing existing images, another investigation, monitoring or surgery would be more appropriate than enteroscopy. These options are not interchangeable; the clinician should explain the trade-off for your particular finding.

What should I ask before agreeing?
Ask the team to explain:
• Whether the plan is examination only, biopsy or treatment.
• Why the proposed insertion route is appropriate.
• What happens if the target cannot be reached.
• Whether admission or a second procedure could be necessary.
These questions reflect the importance of selection, preparation, patient experience and complication monitoring in ESGE’s 2025 quality framework. They do not establish that any particular facility meets that framework. ESGE quality update, 2025

What are the risks and limitations?
Possible complications include bleeding, a tear in the bowel wall and reactions to anaesthetic medicines. Pancreatitis is a recognised risk of the oral approach. Some abnormalities can be missed, or the scope may not reach the intended area. Ask for risks specific to diagnostic examination versus the treatment being proposed; a general percentage may not describe your situation. King’s College Hospital

How should I prepare and plan recovery?
Follow the unit’s written fasting or bowel-preparation instructions. Tell the team about medicines, allergies and previous anaesthetic problems; do not change prescribed treatment yourself. Arrange an escort and clarify whether someone needs to stay with you afterwards. King’s College Hospital
Ask separately about the immediate examination findings and any pending biopsy result. Before leaving, request a written report, a results contact and instructions for follow-up. Your plan should include what happens if the test does not explain the problem.
Severe abdominal pain, a swollen abdomen, vomiting, fever or significant bleeding after the procedure require urgent medical assessment. Do not wait for a travel coordinator’s reply. King’s College Hospital

Planning an assessment in Istanbul
Before booking travel to Turkey, ask the coordination team how to arrange clinical review of your previous endoscopy, capsule, imaging and pathology reports. Confirm the named clinician, facility, available technique and whether the appointment is a consultation or a procedure.
Request a written plan for results, possible admission, contact after discharge and handover to your usual doctor. Do not assume same-day treatment, a fixed flight date or avoidance of surgery.

Frequently asked questions
Does two balloons mean a better result?
The label alone does not establish the best option for you. Ask how the chosen equipment, route and team experience match the target and planned intervention.
Is this the same as capsule endoscopy?
No. A capsule records images. Balloon-assisted enteroscopy uses an endoscope that can permit sampling and selected treatment.
Can it treat any hard-to-reach tumour?
No. Reaching an area does not establish that it can be safely or completely treated endoscopically. Further pathology, imaging or multidisciplinary assessment may be needed.
Can I return home immediately?
Do not assume so. Confirm observation, escort and travel advice with the treating team, especially if treatment is performed.

Have reports for this procedure?
Send them for a confidential specialist review. A coordinator confirms what is missing and when the doctor's view will be ready.
